In order to perform a name change, we need to know the basis of the change and legal documentation authorizing the change must be provided.  Any of the following valid documentation (copies) are accepted:

1. Marriage Certificate (Marriage Certificate or Passport Required)
2. Divorce Decree (Judgment Page)
3. Adoption (Adoptions Papers Required) 
4. Legal Name Change (Court Document with Judgment Page)
5. Correction (Drivers License or Passport Required)
6. Protected Name (Order of Protection Required)
