The Office of Health Services, located on the main campus, stresses preventative health care and health education by encouraging students to actively participate in their own health maintenance that involves making informed choices regarding their care. Emphasis is placed on holistic health promotion and women's issues in health and wellness. The nurse-managed health center is staffed by registered professional nurses, nurse practitioners and a physician.

The professional staff is available for wellness education, health teaching, health counseling, designing and implementing health promotion programs. The office offers an abundance of health-related literature in the Wellness Resource Library free of charge to the college population.

New York State Law requires all college students born on or after January 1, 1957 to present proof of immunity against measles, mumps and rubella. Enrollment to the College will require documented proof of immunity.

HEALTH FORMS

Official enrollment and clearance to register requires that a complete Health Assessment Form which contains New York State mandated immunization requirements be submitted to the Health Services Office. In addition, documentation of health insurance is required.

Students in all schools returning from a leave of absence of one semester or longer are required to update their Health Assessment Form.

  1. School of Arts and Sciences
    Undergraduate students must submit a complete Health Assessment Form at the time of admission

  2. School of Nursing
    All students in the traditional undergraduate school must submit a complete Health Assessment Form at the time of their enrollment and a supplemental form annually.

    Students in the RN, IT and Graduate Programs in the School of Nursing must submit a complete Health Assessment Form the semester before beginning clinical practice in affiliating agencies and supplemental forms annually.


  3. Graduate School Residents
    All resident students must submit a completed Health Assessment Form before residing on campus.

All forms must be submitted and completed by July 15th for the Fall Semester, December 15th for the Spring Semester and May 15th for the Summer Sessions. The entire process is to be completed by mail. Students who "drop off" the form in the Health Services Offices will receive a Health Status Report Form (clearance) after a five-day period. Students who fail to submit a Health Assessment Form by the designated deadlines may be subject to a late fee. Once the form is on file, students will receive a clearance and are eligible to register and receive routine health care as stated above. The Health Services Office Staff is available to provide services to complete all forms.

STUDENT ILLNESS

A student illness form will be completed if requested by the student at the time of the visit and at the discretion of the health care provider.
