Students in the School of Arts and Sciences and undergraduate School of Nursing are required to submit a Health Assessment Form which contains New York State mandated immunization requirements at the time of admission. In addition, School of Nursing students are required to submit a supplemental health form annually. Annual requirements for School of Nursing students must not expire during the semesters in which they are in attendance. Once the form is on file, students will receive a health clearance and are eligible to receive routine health care.

Students in the RN-BSN program, Individual Track, Second Degree and Graduate Programs in the School of Nursing must submit a complete Health Assessment Form before the student practices in a clinical affiliating agency.

Graduate students who reside on campus are required to submit a completed Health Assessment Form. Once the form is completed, they are eligible to receive routine health services.

Students who participate in intercollegiate sports must complete an additional sports physical by the Health Services Office personnel.

All forms must be submitted and completed by July 1st for the Fall Semester,
December 1st for the Spring Semester and May 1st for the Summer Sessions. The entire process is to be completed by mail. Students who “drop off” the form in the Health Services Office will receive a Health Status Report Form (clearance) after a five-day period. Students who fail to submit a Health Assessment Form by the designated deadlines may be subject to a late fee.

The Health Services Office Staff is available to provide services to complete the Health Assessment Form. All students returning from a leave of absence of one semester or longer are required to resubmit their Health Assessment Form.

The Health Assessment Form includes personal and family health history, and a physical examination performed by a physician or nurse practitioner. An immunization record is required to be on file in Student Health Services. The primary purpose of this information is to assure that immunizations are current and to provide a historical basis for the provision of health care.
